Make a payment
From this page you can access the secure payment system used by the Utah Department of Health and Human Services Division of Licensing and Background Checks (DLBC).
All payments received will be processed on the next business day.
A $2.00 processing fee will be charged per transaction. A transaction may consist of multiple payments.
Instructions
If you are already logged in to your provider portal, you must log out before attempting to access the DLBC payment system.
After confirming that you have logged out of your portal, click on the button below to be taken to the DLBC payment system.
You must fill in all required fields, type in the quantity of each payment that you would like to make, input any necessary notes, click total at the bottom, and then click submit to DLBC.
Make sure that you have your card information on hand. Once you click on submit to DLBC you will be taken to another page to input your card information. You will have 10 minutes to input your card information and complete the payment. When 10 minutes have elapsed, your transaction will time out and you will be required to start the process over from the beginning.
