Human services licensing
This page outlines the process of applying for and renewing a license, certificate, and/or approval.
Any personal data requested is collected in accordance with the DLBC privacy notice.
Apply for a license
1. Identify the license you need

2. Read the rules
3. Gather required documents
Before applying, make sure you have all required documents.
Regardless of the license type you are applying for, you will need:
- An outline of the organizational structure of the agency that includes lines of authority, position titles, job descriptions, and the name and contact information for each responsible decision-maker (including any owner, program director, or facility director).
- Contact information includes a mailing address, phone number and email address for each individual.
- A description of the services to be offered and the population to be served.
Additional documents will be needed once you are assigned a licensor. For more information contact your assigned licensor or process specialist.
4. Submit your application
Incomplete applications will delay the process and may require you to resubmit an application with correct and complete information.
License applications expire after 180 days.
5. Next steps, after your application is processed
Once your application has been processed, you will receive a confirmation email. Follow these steps:
- Create a UtahID, which will allow you future access to your OL provider portal. You cannot pay required fees, access your OL provider portal, or submit additional documents until your application has been processed.
- Pay required fees using the official DLBC make a payment page (licenses require a fee, certifications do not).
- Start background checks. Follow the instructions sent to you in the confirmation email to gain access to the Direct Access Clearance System (DACS) to start the background check process. Background checks must be completed for anyone associated with the program that has access to clients or their information.
6. Schedule an on-site inspection
After completing the previous steps and paying required fees, you will be assigned a licensor who will contact you via email (their information is viewable in your OL provider portal). They will request any remaining documents and schedule an initial inspection. Prepare for the on-site inspection by reviewing the rules and inspection checklists beforehand.
- This inspection will take place on-site at your certified or licensed location.
- During this inspection, your licensor will assess for compliance with the rules and requirements that apply to your program.
- Your program must be found in compliance with all rules and requirements before a license can be issued.

- For residential Settings: HCBS Settings Rule Attestation Tool for Residential Settings and HCBS Settings Transition Provider Self-Assessment Tool, Residential: Setting Specific.
- For non-residential Settings: HCBS Settings Rule Attestation Tool for Non-Residential Settings and HCBS Settings Transition Provider Self-Assessment Tool, Non-Residential: Setting Specific.
The following documents are required for all licensed programs:
- Business License
- Fire inspection (if conducted separate from business license)
- Health inspection (or documentation of exemption from local jurisdiction)
- General liability insurance
- Fire insurance (if not included in general liability)
- Professional liability insurance
- Vehicle insurance (if transporting clients as a part of services)
- Floor plan
- Non-discrimination policy
4. Submit your application

7. Wait to receive your license or certificate
Renew a license or certificate
1. Submit a renewal request and pay required fees (up to 90 days before expiration)
- Log into your OL Provider Portal and complete/submit a renewal request.
- Submit the required renewal fees through the make a payment page. Certifications do not have renewal fees at this time
2. Schedule an inspection
- Contact your licensor to schedule your program review and safety inspection.
- An inspection may only be scheduled after the renewal request and applicable fees have been submitted. Your licensorโs contact information is available in your OL Provider Portal.
- Review the rules and inspection checklist available on the DLBC website to prepare for the inspection.
3. Wait to receive your license or certificate
License or certificate change requests
To make any of the following changes, a new application must be submitted. Follow the instructions in the Apply for a license section above.
- A change of location
- A change in the population served
- A change in the regulation type of the program or facility
- An additional license category
- A change that transfers 50% or more ownership or controlling interest to a new owner
To request any of the following changes, submit a change request through the DLBC provider portal:
- Increase or decrease in your licensed capacity
- Change of facility name
- Less than 50% of ownership change
- Change in phone number or email address
- Change in DHHS contract status
- Change in SUMH certification status
- Change in accreditation status
- Change of administrative site where no clients are served
- Change of specialty
