Youth transportation companies
What is a Youth Transportation Company?
Utah Code 62A-2-101(51) defines a Youth Transportation Company as “any person that transports a child for payment to or from a congregate care program in Utah” not including a relative of the child, a state agency, or an employee of the congregate care program (who transports the child to or from the program with which they are employed).
Youth Transportation Programs are required by Utah Code 26B-2-125 opens in a new tab to register with the Office of Licensing.
- Complete the online Youth Transportation Services Registration Application and submit all of the following:
- A copy of your current business license
- Proof of Insurance: Provide proof of insurance with $1,000,000.00 minimum coverage
- No sooner than 4 working days after submitting the application:
- Pay required fees
- Create a UtahID Login
- Log Into Your Office of Licensing Provider Portal
Youth Transportation Programs are required by Office of Licensing Rule R501-1-4(9) opens in a new tab to register with the Office of Licensing and submit background checks for all transporters.
Background checks will be done through Direct Access Clearance System (DACS)
- Submit clearance worksheets opens in a new tab for all Utah transport staff. A copy of the applicant’s ID and social security card MUST be included.
- Make a background check payment here opens in a new tab. The background check fee is $53.25 per applicant. Select your facility and fill in the box for Youth Transportation Services Background Check Fee. You must indicate the name(s) of the person(s) applying for the background check in the details box.
- Fingerprints may be done one of the following ways:
- For those that are in Utah: Obtain a Fingerprint Authorization form from the Division of Licensing and Background Checks and take it to an authorized Livescan location opens in a new tab to do electronic fingerprints.
- For those that are not in Utah: Submit two rolled fingerprint cards (done on cards obtained by the applicant at a local public safety office
or fingerprinting location in the applicant’s state of origin)

In order to renew your registration, you must submit all required items no later than 30 days before your registration expires. Failure to submit a full renewal request, all required documentation, and pay the required fee may result in the need to reapply for a new registration.
- Fill out and submit a Renewal Request via your Office of Licensing Provider Portal.
- Upload the following information through your Office of Licensing Provider Portal via the “Submit Documentation” left menu option:
- A copy of your current business license
- Proof of Insurance: Provide proof of insurance with $1,000,000.00 minimum coverage
- A list of approved Utah drivers including their date of birth.
- Pay required fee
- When all requirements have been submitted and all criteria have been met, your renewed registration will be sent to you by a Process Specialist.
