Completing an adoption fees and expenses report
Utah Code Title 81 Chapter 13 Part 2 Section 217 requires attorneys who arranged the placement of a child adoptee to provide a copy of an affidavit regarding fees and expenses and birth parent services reporting form to the Office of Licensing (OL) within the Division of Licensing and Background Checks (DLBC) on or before 60 days after the day on which the adoption is finalized.
This affidavit must be submitted through the DLBC provider portal. If you do not already have access to the provider portal, you must request access by following the steps in the next section of this page.
Obtaining access to the DLBC provider portal
Submit an access request
Create a UtahID
A UtahID account is required to access the DLBC provider portal. You may create a UtahID account while waiting for your access request to be processed.
You may use an existing UtahID account or create a new one. If you have an existing account, make sure that an least one of the email addresses provided on your request form is the primary address associated with your UtahID.
If you do not have a UtahID, you must create one using one of the email addresses provided on your access request form.

Log in using your UtahID
Allow up to four working days for your access request to be processed. Once your request has been fully processed and you are approved access to the DLBC provider portal, you will be notified.
After being notified that you have access and creating a UtahID account, you will be able to log into the provider portal using the link below. You will log into the portal using your UtahID, though note that you must access the portal using the specific URL provided here. Simply logging into your UtahID account will not grant you access to the provider portal.
