The Office of Licensing is updating the “Submit a Concern” form for all facility types. The revised form will be accessible soon on the dlbc.utah.gov website under the services section.
Key Changes:
- New Category: You can now select “Child Care”, “Health Facility” or “Human Services” as an option.
- No Change: The “Submit a Concern” form will remain the same when selecting “Child Care” and “Human Services” programs. Smart form logic will not apply when selecting these options.
- Smart Form Logic: For “Health Facilities” the form is now dynamic; additional questions will appear based on your previous answers to ensure all necessary details are captured.
