Make a payment
From this page you can access the secure payment system used by the Utah Department of Health and Human Services Division of Licensing and Background Checks (DLBC).
All payments received will be processed on the next business day.
A $2.00 processing fee will be charged per transaction. A transaction may consist of multiple payments.
Instructions
If you are logged in to your Office of Licensing provider portal, you must log out before accessing the payment system. After confirming that you have logged out, click on the button below to be taken to the DLBC payment system.
You must fill in all required fields, type in the quantity of each payment you would like to make, input necessary notes, click on "Total", and then click "Submit to DLBC".
Once you click on "Submit to DLBC" you will be taken to another page where you will have 10 minutes to input and submit your payment information. When 10 minutes have elapsed, your transaction will time out and you will be required to start the process over.
