Background Check Instructions


If you recently submitted an application for a new child care program, allow up to four business days before submitting any background check requests for your program.

Start a Background Check Request

Select the button below to start a new background check request.

Adults must complete and submit their own form. Parents or guardians may complete a request form on behalf of their child.

Authorize the Background Check

Enter your name, phone number, and email address in the "AUTHORIZATION AND RELEASE" section.

Enter Individual Information

Fill all required fields in the "COVERED INDIVIDUAL INFORMATION" section.

Complete Final Fields

Fill the fields in the "CONTACT INFORMATION" section, fully read through the disclaimer and check the box at the end to acknowledge that you received and read the FBI privacy act statement, and select "Submit to Provider" to complete your background check request.

Authorize the Request

After submitting a background check request, the provider (or owner) of the facility you selected in step 3 must log into their provider portal and authorize your background check request.

Unless this step is taken, your background check request will not be processed by the Office of Background Processing (OBP).

Schedule a Fingerprinting Appointment

Fingerprints are required if:

  • You are 18 years old or older and have not previously submitted fingerprints to OBP for a background check using the Next Generation FBI fingerprint or rap-back system.
  • You are 18 years old or older, and have resided outside of Utah since your last fingerprints were submitted to OBP.
  • You are 18 years old or older, and have not been associated with any child care facility in Utah for the past 180 days.
  • You are 16 years old or older and not listed as a household member (individuals with dual roles such as caregiver and household member must submit fingerprints).

If you are required to submit fingerprints to OBP as part of your background check, you must schedule an appointment with an approved agency to have your fingerprints taken and sent to OBP.

Your provider must download and print a physical copy of your fingerprint form from their provider portal. You must take this form with you to your fingerprinting appointment.

You may have your fingerprints captured by any of the agencies listed in the document linked below. If you live near Salt Lake City, you may also schedule an appointment to have your fingerprints captured by OBP directly.

Pay Required Fees

You or your provider must pay the associated background check and fingerprint processing (if required) fees to DLBC.

Note that the agency where you have your fingerprints captured may also charge a fee. This fee is separate from any fees owed to DLBC, which must be paid in full regardless of any fees paid to other agencies during the background check process.

Await Your Results

Before being left unsupervised with children, counting in the caregiver-to-child ratio, or acting as caregiver, you must received notice from OBP that you have been determined eligible to be involved in child care.

General Information

Approval will be denied for any Covered Individual with a felony or misdemeanor A conviction, plea in abeyance, conversion, no contest, or diversion. Approval may also be denied for specific misdemeanor B or C convictions, such as offenses against a person, pornography, prostitution or any type of sexual offense, simple assault, lewdness, child abuse, contributing to the delinquency of a minor, selling or supplying alcohol to a minor, and animal cruelty. Individuals listed on the Utah or National Sex Offender Registry will also be denied.

If there is an error on a record or if the record can be expunged, it is the Covered Individual’s responsibility to resolve the matter by contacting the Utah Department of Public Safety, Bureau of Criminal Identification. When the matter is resolved, the Covered Individual must submit a new Background Check form and legal documentation of the expungement, dismissal, etc.

All information regarding the Covered Individual’s background check will be kept confidential and no confidential details regarding the check will be released or disclosed over the phone. The Covered Individual and the provider will be notified if the background check is not approved.

A Background Check Card will be issued to the approved Covered Individual and sent to the facility.

A Covered Individual can use their current Background Check Card to work or volunteer at any child care facility.

Information for Out-of-State Background Check Agencies


Utah participates in the FBI National Fingerprint File (NFF) program, meaning it is unnecessary for an out of state agency to conduct an FBI fingerprint check and search through Utah's criminal history record repository. Agencies conducting an FBI fingerprint check will only need to request records from Utah directly for noncriminal justice purposes.

Out of state agencies may request a release of information from the Utah child abuse central registry by completing and following the instructions contained in this authorization and consent form.
